Chronic Disease Management 

As General Practitioners we often get requests for referrals to the physiotherapist or another allied health providers. The Chronic Disease Management Plan (formerly Enhanced Primary Care or EPC) enables GPs to plan and coordinate the health care of patients with chronic or terminal medical conditions with at least two other health providers such as a Consultant, physiotherapist or podiatrist.

You have to meet specific criteria to be eligible for a Chronic Disease Management Plan. A chronic medical condition is one that has been present for six months or longer, for example, asthma, cancer, cardiovascular disease, diabetes, musculoskeletal conditions and stroke.

We bulk bill our Chronic Disease Management Plans but they require a specific type and length of appointment. Please call our reception for further information.

Vaccinations – Routine childhood vaccinations are provided at our clinic and are all bulk billed. Please bring your Green Book or vaccination record to your appointment. We can also provide catch-up vaccinations for any childhood vaccinations that have been missed.

Young Adult Health – We respect a person’s autonomy over one’s health and provide a confidential service to teenagers. The only need to share confidential information would be if a person’s health or life was at serious risk.
